Grievance checklist and deadline tracker

A grievance checklist and deadline tracker for union stewards: one grievance per form, with the intake questions, the six Ws, the provisions and remedy, the pre-filing checks and a due date for every stage. Built from the SEIU Local 521 intake form and the PSAC grievance file checklist. Use it three ways: fill it in on screen below (nothing you type is sent anywhere; it stays in your browser until you clear it), download the fillable PDF to keep on a phone or laptop, or print the two-page version. The procedure it tracks is explained in The union grievance procedure, explained step by step, and the filing step in How to file a grievance.

Both include whatever you have filled in below. The PDF stays editable, so you can keep working in it on a phone or laptop; if you have typed nothing, you get a blank form either way.

1. Intake

Fill this in at the first conversation. Dates decide grievances; get them first.

2. The six Ws

3. The case

4. Before you file

5. The clocks

One line per stage. Write the due date the day the previous stage ends, not the day you remember.

Why the dates come first

A grievance filed a day late can be dismissed without the merits being heard, and the clock usually runs from the event or from when the member should have known, not from the day the steward hears about it. That is why this form asks for the incident date and the knowledge date before anything else, and why section 5 gives every stage its own due date, filed-on date and employer-answer date. The deadlines in real agreements run from 10 days to 35 days for the first step; the comparison is in the clocks section of the procedure guide.

This checklist is general information, not legal advice. Your collective agreement and your union's advice govern the steps and deadlines that apply.
